LDAP_THREAD_DEBUG revamping, cleanup, bug fixes.
Expand usage struct with magic numbers and state info.
Add options "noreinit", "nosync", "nomem", "threadID".  Remove "dupinit".
Detect address changes.  Turn off thread ID tracking by default.
