Fix GCC7 warnings about switch-case fallthroughs

GCC7 with -Wextra warns about switch-case blocks which fallthrough with
"this statement may fall through [-Werror=implicit-fallthrough=]". This
can be fixed by adding "special" comments: "/* fallthrough */".
