- Fix more print_name()s for printing device name.

- Modify open_dev to try 10 times every 6 seconds to
  open the device if it gets an I/O error (meaning no
  volume mounted). This gives a bit of settling in time
  for an autochanger and avoids spurious messages.
- Change all yes/no to yes|no in the manual.
- Fix win32 create_file.c typo.
- Fix a typo in an error message.
