  12 - Fixed create_mysql_database, which I recently accidently broke.
  13 - Disallow zero length volume names in Console program.
  14 - Create alter_mysql_tables and alter_sqlite_tables for conversion
  15   from 1.22 table format to 1.23 table format.
  16 - Fix recursive call to Emsg() if could not open output file. Done by
  17   sending output to stdout.
